Intuitive Surgical grew Q2 2026 revenue 19% to $2.89 billion on 15% da Vinci procedure growth and 468 system placements, with operating margin up more than three points to 33.6% — helped by a one-time $35.9 million tariff refund that will not repeat.

Procedure volume, not price, is still doing the work — and a tariff refund flattered the margin
Intuitive Surgical makes the da Vinci robotic surgery systems that hospitals buy (or lease) and the single-use instruments that get consumed every time a surgeon operates with one. The economics are closer to a printer-and-ink business than to a capital-equipment business: the machine is the door-opener, but most of the money arrives afterwards, procedure by procedure.
That model held up in the June 2026 quarter. Revenue rose 19% to $2.89 billion, and the driver was volume: surgeons performed roughly 889,000 da Vinci procedures, 15% more than a year earlier. Operating income — profit from running the business, before interest and tax — grew faster than revenue, up 31% to $972 million, lifting operating margin from 30.5% to 33.6%.
But about a third of that margin gain came from something that will not repeat. Intuitive booked $35.9 million of refunds on tariffs it had paid in prior periods, after the U.S. Supreme Court ruled in February 2026 that the IEEPA-based tariffs were unlawful. Those refunds land as a reduction of cost of revenue, so they show up as margin. Strip them out and the picture is still good — just less good.

All figures from the Q2 2026 Form 10-Q. "Recurring revenue" is the company's own definition: instruments and accessories, service contracts, and operating-lease revenue — i.e. everything except outright system sales.
What actually drove the 15% procedure growth
The filing is specific about where the cases came from, and the two halves of the business look different.
U.S. procedures grew about 12%, down from 14% growth a year ago. Management attributes it to general surgery — "most notably cholecystectomy, hernia repair, and appendectomy procedures" (gallbladder removal, hernia repair, appendix removal) — plus gynecology. Two drags are named: a high-single-digit decline in U.S. bariatric (weight-loss surgery) procedures, which the company says are only about 2% of total da Vinci volume, and a softer, hedged one — management says it believes U.S. growth was "modestly negatively impacted by the expiration of enhanced ACA premium subsidies," and that the extent of the continued impact "remains uncertain." The bariatric decline is the visible edge of GLP-1 weight-loss drugs substituting for surgery; at 2% of volume it is not yet a threat to the model, but it is the one procedure category going backwards.
Outside the U.S., procedures grew about 20%, with strength named in India, Italy and the United Kingdom, led by colorectal, hernia, gallbladder, prostatectomy, partial nephrectomy and hysterectomy cases. Both U.S. and international growth rates decelerated versus the prior-year quarter (12% vs. 14%, and 20% vs. 23%) — a wider installed base makes each incremental percentage point harder, but the deceleration is real and consistent across geographies.
Instruments and accessories revenue grew 18%, slightly faster than the 15% procedure growth. The filing explains the gap: higher revenue per procedure, driven by the instrument mix on the newer da Vinci 5 and single-port SP systems, plus 36% growth in Ion lung-biopsy procedures. That is the healthiest possible reason for the gap — richer instrument use per case rather than list-price increases.
Systems: more placements, but a growing share are leases
Intuitive placed 468 da Vinci systems, up 18% from 395, of which 246 were the fifth-generation da Vinci 5 (up from 180). Average selling price on systems that were actually sold rose to roughly $1.59 million from $1.50 million, which the company attributes to favorable product mix from da Vinci 5, partly offset by unfavorable geographic mix.
Two things complicate the headline placement number:
54% of placements were operating leases, up from 49% a year ago. Under a lease, revenue is recognized over time (or as procedures are performed) rather than upfront, so a rising lease share mechanically holds back reported systems revenue even when unit demand is strong. Operating lease revenue was $262 million, up from $214 million, of which $169 million was usage-based. Lease buyouts — customers exercising an option to purchase a system they had been leasing — contributed $56 million versus $30 million, a lumpy item management explicitly warns will fluctuate.
144 of the 468 placements involved trade-ins, up from 83. Customers swapping fourth-generation systems for da Vinci 5 is good for the installed-base refresh, but it is a different, lower-quality source of placements than net new hospital capacity, and the company flags "a declining number of older generation systems available for trade-in transactions" as a future constraint on placements.
China is the clear soft spot. Management says that in Q2 2026, competition from domestic robot manufacturers, industrial-policy measures, and the government's healthcare governance campaign "contributed to fewer systems being placed in China than we anticipated." Separately, China's National Healthcare Security Administration issued guidelines in the quarter establishing a national framework capping what hospitals can charge for robotic-assisted surgery, to be implemented by all provinces "in the upcoming quarters." The company says the limits implemented so far have not been material because only a small part of its China installed base sits in affected provinces — but that is a statement about today's footprint, not about the national rollout, and it is the single item in this filing with the clearest path to hurting instruments revenue later.
Margins: how much of the improvement is real
Gross margin — the share of revenue left after the direct cost of making and servicing the product — rose to 67.8% from 66.3%. The filing names the causes for the quarter: IEEPA tariff refunds, lower logistics costs, and fixed-overhead leverage (spreading factory costs over more units), partly offset by higher intangible asset amortization.
The tariff line deserves unpacking, because it cuts both ways:
Tariffs and other trade measures charged to cost of revenue were $20.8 million in the quarter ($48.8 million year to date) — an ongoing cost.
Refunds of previously paid IEEPA tariffs were $35.9 million ($27.5 million in product cost, $8.4 million in service cost) — a one-off credit, recognized only when realizable.
Net, tariffs were about a $15 million benefit to cost of revenue this quarter instead of a cost — worth roughly half a point of gross margin that reverses the moment refunds stop. And the underlying exposure has not gone away: a 10% Section 122 tariff replaced the struck-down IEEPA tariffs in February 2026 and remains in place pending appeal, Intuitive manufactures the large majority of its instruments in Mexicali, Mexico (currently exempt under USMCA) and most of its endoscopes in Germany (not exempt), and the company expects continued adverse cost impact on da Vinci Xi systems sold into China.
Below the gross line, cost discipline was genuine. Selling, general and administrative expense grew only 10% against 19% revenue growth, falling from 23% to 21% of revenue. R&D grew 18%, holding steady at 13% of revenue — management says it expects R&D to keep rising. Share-based compensation inside operating income was $213 million, up from $200 million; intangible-related charges jumped to $24.0 million from $4.8 million following the March 2026 distributor acquisition.
EPS grew 26.5%, faster than the 24.3% net income growth, because the diluted share count fell to 357.3 million from 364.1 million — Intuitive repurchased 0.9 million shares for $0.38 billion in the quarter and $1.44 billion year to date. That is roughly two points of the EPS growth bought rather than earned.
Takeaway: The 3.1-point operating margin expansion is the headline, but roughly half a point of it is a non-repeating tariff refund and a further slice is buyback-driven at the EPS line. What is durable is the mix: 85% of revenue is recurring, instruments revenue grew faster than procedures because each case now uses richer da Vinci 5 and SP instruments, and SG&A grew at half the rate of revenue. The thing to watch is not this quarter's margin — it is that procedure growth decelerated in both the U.S. (12% vs. 14%) and internationally (20% vs. 23%), and that China's new national price caps on robotic surgery are still ahead of the company, not behind it.

The 27.6% first-half international growth rate is not a run-rate. In Q2 alone, non-U.S. revenue grew 18%, essentially in line with the company overall — the half-year figure is inflated by a much stronger Q1. Three identifiable reasons, in the filing's own words: currency moves helped non-U.S. revenue by $40 million in the first half but only $4 million in Q2 (i.e. almost the entire foreign-exchange tailwind landed in Q1); management cites "customer buying patterns" as a contributor to first-half instruments growth that it does not repeat for the quarter; and on 1 March 2026 Intuitive paid roughly $533 million to buy out its da Vinci and Ion distributors in Italy, Spain, Portugal and associated territories, taking those markets direct. The filing does not quantify that acquisition's revenue contribution, but selling direct rather than through a distributor mechanically raises reported revenue per unit in those countries. Treat the Q2 18% as the cleaner read on international demand.
One more first-half distortion worth naming: net income grew 20.8% while operating income grew 38.3%. The gap is tax. The effective tax rate was 17.3% in the first half of 2026 versus 8.8% a year earlier, because excess tax benefits from employee share awards — a credit whose size depends on Intuitive's own stock price and vesting schedules — fell to $90.6 million from $178.3 million. The prior-year period, not this one, was the unusual one.
Cash generation was strong: $1.97 billion from operations in the half, against $1.65 billion of net income, despite $515 million going into inventory (partly to build leased-system fleets and buffer supply-chain risk). Cash and investments still fell to $8.63 billion from $9.03 billion at year-end, spent on the $1.44 billion of buybacks and the $528 million acquisition.
What management says is coming
The 10-Q contains no revenue or EPS guidance — Intuitive gives those on its earnings call, not in the filing — but it does commit to several specific cost directions:
Depreciation expense will continue to increase through the rest of 2026, following elevated 2025 capital spending on manufacturing capacity.
Intangible amortization will be higher in the remainder of 2026 than in 2025, because of the March distributor acquisition. It was already $24.0 million in Q2 versus $4.8 million a year earlier.
R&D expense will continue to increase, reflecting a broader product pipeline.
Customers are expected to stay cautious about overall capital spending, with lower public healthcare funding named in certain markets.
Read together, those point to operating margin expansion being harder to sustain from here than the Q2 print implies: two of the three named cost lines rise mechanically, and the tariff refund does not recur. The offsetting force is the one that matters most over years rather than quarters — the installed base is now about 11,710 da Vinci systems (up 12%) plus about 1,096 Ion systems (up 21%), utilization per system rose 3%, and 85% of revenue arrives regardless of whether a hospital signs a new capital purchase order this quarter. The reasonable expectation is mid-to-high-teens revenue growth with margins roughly flat to modestly up on an underlying basis, with two specific risks that would break that: the China pricing framework rolling out beyond the small share of the installed base it currently touches, and U.S. procedure growth decelerating further if bariatric-style substitution spreads beyond 2% of volume.
