Labcorp Q3 2026: revenue +6.9% and adjusted EPS +14.5%, but GAAP pretax profit fell 2.7%
Q3 · Fiscal year 2026 · Published by Pham Hop
Labcorp's Q3 2026 revenue rose 6.9% to $3.81B (5.5% organic) and adjusted EPS rose 14.5% to $4.78, but GAAP pretax income fell 2.7% as excluded costs jumped 44%; diluted EPS rose 3.3% to $3.22 on a lower tax rate and fewer shares, and full-year guidance was raised.
At a glance
- Organic revenue growth of 5.5%, up from 4.2% in Q2. Biopharma Laboratory Services (BLS) grew 9.6% organically and its Central Labs business 11.5%, so the drug-trial side is now growing faster than routine testing.
- Adjusted operating margin of 15.0% vs. 14.4%, but GAAP operating margin of 10.6% vs. 11.1%. Both cannot be the "real" trend. The gap widened because excluded costs rose 44%, and some of them come back every year.
- Operating cash flow of $563.8M, 2.1 times net income. More than all of the $131.7M increase in free cash flow came from paying suppliers later: payables added $58.0M of cash this quarter, after using $123.8M a year earlier.
Adjusted EPS +14.5% excludes $167.9M of costs (up 44%), incl. LaunchPad costs excluded every year; GAAP pretax income fell 2.7% and EPS +3.3% came from tax rate and buybacks
The reported figures look better than the underlying business: one-off gains, a prior-year charge that did not recur, a lower tax rate, buybacks or reserve releases did much of the work.
In this report
Labcorp's revenue rose 6.9% to $3.81 billion in the third quarter of 2026. Of that, 5.5 points was organic growth, meaning growth from the existing business before acquisitions and currency. Adjusted EPS, the company's preferred profit-per-share figure, rose 14.5% to $4.78, and management raised its full-year revenue and adjusted EPS guidance. The GAAP figures, the ones prepared under standard accounting rules, tell a weaker story. Operating income rose only 1.4% to $402.1M and pretax income fell 2.7%, because the costs Labcorp leaves out of its adjusted numbers jumped from $116.7M to $167.9M. Reported diluted EPS still rose 3.3% to $3.22, but only because the tax rate fell and there were 2.5% fewer shares. Cash flow is the strongest part of the quarter: free cash flow rose 47% to $412.2M.

Key figures
| Metric | Q3 2026 | Q3 2025 | YoY Change |
|---|---|---|---|
| Revenue | $3,810.0M | $3,563.5M | +6.9% |
| Organic revenue growth | +5.5% | — | — |
| Gross margin | 29.1% | 28.8% | +30 bps |
| Operating income (GAAP) | $402.1M | $396.6M | +1.4% |
| Operating margin (GAAP) | 10.6% | 11.1% | −60 bps |
| Adjusted operating margin | 15.0% | 14.4% | +60 bps |
| Net earnings attributable to Labcorp | $262.8M | $261.1M | +0.7% |
| Diluted EPS (GAAP) | $3.22 | $3.12 | +3.3% |
| Adjusted EPS | $4.78 | $4.18 | +14.5% |
| Diluted shares (weighted average) | 81.6M | 83.7M | −2.5% |
| Free cash flow | $412.2M | $280.5M | +47.0% |
| Diagnostics requisition volume (organic) | +3.1% | — | — |
| BLS backlog | $8.92B | — | +3.9% |
| BLS book-to-bill (trailing 12 months) | 1.10 | — | — |
Basis points (bps) are hundredths of a percentage point, so 60 bps is 0.6 points. Book-to-bill compares new orders with revenue. A figure above 1 means the order book is growing faster than the company is working through it.
The two businesses
Labcorp has two segments. Diagnostics Laboratories (Dx) runs the blood and tissue tests doctors and hospitals order and is about three-quarters of revenue. Biopharma Laboratory Services (BLS) runs the lab work behind drug companies' clinical trials.
| Segment | Revenue | Growth | Organic growth | Adjusted op. margin | Year ago |
|---|---|---|---|---|---|
| Diagnostics Laboratories | $2,948.9M | +6.5% | +4.4% | 16.7% | 16.3% |
| Biopharma Laboratory Services | $866.7M | +8.5% | +9.6% | 18.3% | 16.5% |
Diagnostics. About a third of Dx growth was bought. Acquisitions added 2.2 points, leaving 4.4% organic. That organic figure splits into 3.1% more requisitions (test orders) and 1.3% better price and mix, meaning more revenue per order. Most organic growth came from doing more tests, not from charging more. The adjusted margin rose 40 bps, which Labcorp attributes to "organic growth and operating efficiencies."
Biopharma Laboratory Services. Organic growth (9.6%) was higher than reported growth (8.5%) because Labcorp is deliberately shrinking its Early Development business (pre-clinical work), which subtracted 0.8 points. Central Labs grew 11.5% and Early Development 0.4%. Shedding the weaker work is also what lifted the segment's adjusted margin 180 bps to 18.3%. The order book improved: backlog is up 3.9% to $8.92B and trailing book-to-bill is 1.10. Of that backlog, $2.82B is expected to turn into revenue over the next 12 months.
Takeaway: The business grew faster this quarter, at 5.5% organic, led by Central Labs and a stronger BLS order book. The 14.5% adjusted EPS headline overstates how much of that reached shareholders. GAAP pretax income fell, because the costs Labcorp excludes grew from $116.7M to $167.9M. Without a lower tax rate and 2.5% fewer shares, GAAP EPS would have been flat at about $3.12. The strong cash quarter is also partly timing. The revenue picture is better than in Q2. The profit picture is only as good as you believe the excluded costs are temporary.
What the headline numbers hide
The adjusted-vs-GAAP gap widened, and part of it is recurring. Labcorp's adjusted operating income ($570.0M, +11.0%) adds back five kinds of cost:
| Excluded item | Q3 2026 | Q3 2025 |
|---|---|---|
| Amortization of acquired intangibles | $78.7M | $69.5M |
| LaunchPad costs (systems and process programs) | $28.6M | $13.7M |
| Acquisition and disposition costs | $23.0M | $10.8M |
| Restructuring and other charges | $15.9M | $5.9M |
| Other | $21.7M | $16.8M |
| Total | $167.9M | $116.7M |
Amortization is a non-cash accounting charge for intangible assets acquired with past purchases. Excluding it is standard practice. The other items are harder to call one-offs:
- LaunchPad costs are excluded every year: $51.0M in the first nine months of 2025 and $53.3M in the first nine months of 2026. Labcorp describes them as costs of "implementation of systems, consolidation of processes, and consulting."
- Acquisition costs come with buying businesses. Labcorp closed one deal this quarter (MLM Medical Labs) and signed three more after quarter-end (parts of BioReference Health, Myriad Genetics' prenatal testing business, and Millennium Health). A company that buys businesses every quarter will keep incurring these costs.
Without amortization, excluded costs nearly doubled, from $47.2M to $89.2M. That is why GAAP operating margin fell 60 bps while adjusted margin rose 60 bps.
Where the cost growth sits. Gross margin, the share of revenue left after the direct cost of running tests, improved from 28.8% to 29.1%. Selling, general and administrative (SG&A) expense, the overhead line, rose 10.5% to $611.4M, faster than revenue. It went from 15.5% to 16.0% of revenue. The added-back LaunchPad, deal and "other" costs largely sit in this line. Unallocated corporate expense (head-office cost not charged to either segment) rose 16.3% to $80.6M. Without the 10-Q, the release does not break SG&A down further.
Below operating income, three small items all went the wrong way.
- Interest expense rose to $59.1M from $56.0M.
- The loss from the minority stake in SYNLAB grew to $10.9M from $5.0M. Labcorp also excludes this from adjusted EPS.
- Investment income fell to $1.4M from $3.5M.
EPS growth came from tax and buybacks, not operations. Pretax income fell 2.7% to $328.0M. The effective tax rate dropped from 22.4% to 19.8%. At last year's rate, net earnings would have been about $254M and diluted EPS about $3.12, unchanged. Diluted share count fell 2.5% to 81.6M after $451.8M of buybacks earlier in the year (none in Q3 itself), and that accounts for the rest of the 3.3%. The same share-count effect adds about 2.6 points to adjusted EPS growth: adjusted net income rose 11.6%, and adjusted EPS rose 14.5% because the same profit was split over fewer shares.
Cash conversion was strong, but partly timing. Operating cash flow of $563.8M was 2.1 times net earnings, and free cash flow rose $131.7M. The cash-flow statement shows a $181.8M swing in accounts payable (money owed to suppliers): +$58.0M this year vs. −$123.8M a year ago. That swing alone is larger than the free-cash-flow increase. Inventory also released $25.3M. Receivables moved the other way. They grew $70.2M in the quarter and $317.3M year-to-date, and stand at $2,412.9M vs. $2,103.8M at year-end (+14.7%), against 9-month revenue growth of 6.2%. Part of that will be from acquired companies' balances, which the release does not separate. It is still the line to check in the 10-Q.
Balance sheet. Cash fell to $261.8M and total debt was $5.85B at quarter-end. $255.5M went on acquisitions and $58.2M on dividends. The three signed deals will need more cash once they close.
On balance the quarter is flattered. The headline adjusted EPS growth of 14.5% excludes $167.9M of costs, up 44%, including LaunchPad costs that recur every year. GAAP pretax income fell 2.7%. The 3.3% GAAP EPS gain came entirely from a lower tax rate and fewer shares.
Did last quarter's read hold up?
Our Q2 2026 report raised three questions:
- Second-half free cash flow. H2 needed $0.86–0.98B to hit the full-year target. Q3 delivered $412.2M, leaving $443–563M for Q4. That is on track, though payables timing helped.
- BLS orders. We noted that a backlog up only 0.2% and a 1.03 book-to-bill would cap BLS growth. Both have improved, to +3.9% and 1.10.
- Personnel costs squeezing Diagnostics margins. This did not happen: Dx adjusted margin rose 40 bps.
Guidance and outlook
Management raised 2026 guidance (exchange rates as of September 30):
| Previous | Updated | |
|---|---|---|
| Enterprise revenue | $14,710–14,827M (+5.4% to +6.3%) | $14,779–14,887M (+5.9% to +6.7%) |
| Diagnostics revenue growth | +5.3% to +6.0% | +5.5% to +6.2% |
| BLS revenue growth | +5.5% to +6.5% | +7.4% to +8.4% |
| Adjusted EPS | $18.10–18.55 | $18.30–18.70 |
| Free cash flow | $1,240–1,360M | $1,240–1,360M (unchanged) |
The updated BLS range includes 1.3 points of currency benefit, and the enterprise range 0.3 points.
Our read. Nine-month revenue was $11,078.7M, so the range implies Q4 revenue of about $3,700–3,808M, or +5.2% to +8.3% over Q4 2025's $3,516.1M (derived from 2025 revenue of $13,952M). The top end needs Q3's pace to continue plus acquisition revenue. Nine-month adjusted EPS of $14.02 implies Q4 adjusted EPS of $4.28–4.68, below Q3's $4.78. That leaves room even if Q4 brings fewer working days and more deal costs.
The free-cash-flow target was left unchanged even after a strong Q3. One reading is that management does not expect the payables benefit to repeat.
The BLS raise is backed by the better order book. For Diagnostics, the 3.1% organic volume growth is the figure to watch, since acquisitions will make reported growth look higher through 2027. The other figure to watch is whether excluded costs stay near $90M a quarter, excluding amortization, as the three new deals are integrated.
